ISP investigation leads to arrest of Choate Mental Health employee

ANNA (WSIL) -- An Illinois State Police (ISP) Investigation has led to the arrest of a Choate Mental Health Developmental Center employee.

ISP arrested Teresa Smith, 57, of Pulaski, Illinois.

On February 5, 2020, the ISP Division of Internal Investigation was requested by the Illinois Department of Human Services (IDHS) staff to investigate a battery allegation by a Mental Health Technician on a resident at Choate.

After a lengthy investigation, Union County State's Attorney Daniel Klingemann's Office issued a summons for Smith, requiring a court appearance on December 4, 2020.

This case has been turned over to the Union County State’s Attorney and no additional information is available for release by ISP.
